Registration Checklist

Here is information you will need to complete your registration form. Compiling this info beforehand will help you avoid interruptions and possible loss of data while you register on-line.

  • (  ) A brief description of your professional experience that you believe qualifies you to be appointed as a conservator, guardian or trustee.
  • Your educational background. If listing any colleges, you will need to provide

This on-line registration process is for use by all private conservators, guardians and trustees who are required to participate in the Statewide Registry of Private Conservators, Guardians and Trustees.

Here are some important things to keep in mind for this registration process:

Time Limits. You will need to complete the registration process at a single session. You have a maximum of 30 minutes per screen to complete the entry of your information. If you exceed this time limit, you will lose all the data you entered and must start over.

BEFORE YOU START, Gather the Info You Will Need. To avoid interruptions and possible loss of data, take a moment to compile the data you will need for your registration form. For your convenience, we offer a Registration Checklist.

You can only proceed forward on the on-line registration form. If you need to edit a previous entry, you may do so on the summary REVIEW YOUR INFORMATION page. This appears immediately after the page asking about your education.

Print, Sign and Enclose Registration Fee and Documentation of Educational Degrees Claimed. While using this online form transmits your data electronically to the statewide registry, there are additional steps to complete the registration process.

You must print, sign and mail a copy of the registration information with payment of your registration fee and documentation of educational degrees claimed to the Statewide Registry of Conservators, Guardians and Trustees.

Documentation can be either a TRANSCRIPT or A LETTER from the educational institution that verifies the degree(s) claimed. Verification is not needed if have chosen not to list a degree you may have. For example, if you graduated from college with a Bachelor of Science degree and law school, but choose only to list your law degree, you do not need to verify your college degree. However, if you claim both degrees, you must submit verification of each degree.
